Vaccination Instructions

To make your visit as smooth and safe as possible, please follow these instructions when you arrive at the pharmacy.

1. Please download, print, and complete the intake form. Click here to download the Intake Form

2. Please arrive promptly during your vaccination window.

3. Please come to the drop off window to check in.

4. Please provide a completed intake form (from Step 2) or we will provide a form for you to fill out.

5. Please bring your insurance cards for billing purposes.

6. Please read, print, complete, and bring the following forms to your appointment:

   • COVID Vaccine Consent Form (Print, Complete, & Bring to Appointment)

   • Moderna Emergency Use Authorization Form (Read Only)

   • Johnson & Johnson Emergency Use Authorization (Read Only)

   • Pfizer Emergency Use Authorization Form (Read Only)

   • Pediatric Pfizer Emergency Use Authorization Form (Read Only)

   • V-Safe Information (Read Only)

7. Pharmacy staff will instruct you on when you can enter our vaccination room.

8. Receive vaccination.

9. Please wait in the pharmacy for 15 minutes and maintain social distancing.

10. After 15 minutes, please check in with our vaccination team before you leave.
