Vaccination Instructions

To make your visit as smooth as possible, please follow these instructions when you arrive at the pharmacy.

1. Please download, print, and complete the intake form where available. Or fill one out at the pharmacy when you arrive.

2. Please arrive promptly during your vaccination window.

3. Please come to the drop off window to check in.

4. Please provide a completed intake form (from Step 1) or we will provide a form for you to fill out.

5. Please bring your insurance cards for billing purposes.

6. Pharmacy staff will instruct you on when you can enter our vaccination room.

7. Receive vaccination.

8. Please wait in the pharmacy for 15 minutes.
